Presentation of Military Discharge Document for Recording Form

Effective January 1, 2004, military discharge papers must be submitted in person by an authorized party. This form must be completed and signed by the person presenting the military discharge paper. The Land Records Committee recommends that this form be permanently retained to provide documentation that the party who presented the document for recording was authorized to do so.

Request for Copy of Military Discharge Document Form
Each person requesting a copy of a military discharge record that is subject to restricted access must complete this form. The Register of Deeds must retain completed request forms for a period of one year from the date of application. Completed request forms are not public records.

Authorized to Record/Request Copy of Military Discharge Document Form

This form is used to give an authorized agent or representative the authority to record or request a copy of a military discharge record. This is subject to restricted access. This form must be signed by the veteran, the veteran’s widow or widower, or by the veteran’s executor. This form must be notarized. This form must accompany the Presentation of Military Discharge Document for Recording form or the Request for Copy form if either form is signed by a person who indicates that they are an authorized agent or representative of the veteran, veteran’s widow or widower, or veteran’s executor.

A. No. There is no fee for filing a military discharge. Certified copies of military discharge documents are free to military veterans.

Q. What do I need to record a military discharge?
A. To record a military discharge, the entire original DD214 is required.
